1. Not medical advice

Teck Wellness is a technology platform that screens patients for GLP-1 program qualification and routes qualified patients to independently licensed clinics. We do not provide medical care and no doctor-patient relationship is created by using this platform.

2. Eligibility

You must be at least 18 years old to use the patient intake form. Clinic partners must be licensed to provide care in the jurisdictions they serve.

3. Acceptable use

You will not submit false information, attempt to access another user's data, or interfere with the operation of the platform.

4. Clinic partner terms

Clinic partners are engaged under a separate Master Services Agreement and Business Associate Agreement that govern payment, data handling, and HIPAA obligations.

5. Disclaimers

The platform is provided "as is" without warranty of any kind. Teck Wellness is not liable for the medical decisions made by clinic partners.
